PROPOSED CHANGES The SPA for Social Security Act section 1945 Health Home authority will add a new benefit to the state plan to assist individuals with complex behaviors who have multiple diagnosis of Intellectual Disabilities/Serious Mental Illness or Autism, who are experiencing crisis. These individuals have an acuity which exceeds the existing level of traditional community services and have a need for a more specialized plan of care delivered through an interdisciplinary team of healthcare professionals. The Intellectual Disabilities/Serious Mental Illness (ID/MI) Health Home multi-disciplinary team approach to care includes access to comprehensive care management, care coordination, health promotion and provides for comprehensive transitional care, individual and family support services, and referral to community and social support services. The ID/MI Health Home team attaches to the individual and transitions with them across multiple systems of care and serves as the consistent bridge to treatment and supports the individual needs to be successful. The SPAs for Recovery Resilience and Support (RRAS) Services will add a benefit to the Basic and Enhanced Alternative Benefit Plans under Social Security Act section 1937 authority. RRAS Services consist of medical, behavioral health, and support services tailored to the individual based on the ID/MI Health Home plan of care. Services are delivered by ID/MI Health Home partners through a linkage agreement. RRAS providers are responsible for:  • Coordinating with the ID/MI Health Home team. • Carrying out the recommended treatment plan. • Providing RRAS services in accordance with the plan and within the scope of practice for their respective disciplines. • Tracking the care delivered and documenting outcomes. • Utilizing appropriate electronic technology for documentation. The Department intends to submit these State Plan Amendments to CMS with requested effective dates of January 1, 2024.
